“I am now no more than a pile of blood, bone, and meat that is unhappy”*…

Most of us are familiar with the placebo effect. Dr. Michael H. Berstein explains the “nocebo”…

The term “nocebo effect” derives from the Latin word nocere, which translates roughly as “to harm” (as in the Hippocratic injunction, primum non nocerefirst, do no harm). Whereas the better-known placebo effect is typically positive (the alleviation of pain or malaise through treatments that otherwise have no inherent therapeutic value); the nocebo effect is negative, often manifesting as headache, skin irritation, or nausea.

No surprise, then, that the nocebo effect has been called “the placebo effect’s evil twin.” It can be more formally summarized as “the occurrence of a harmful event that stems from conscious or subconscious expectations.” Or, more simply: When you expect to feel sick, you are more likely to feel sick.

Of course, human expectations come up in all sorts of banal, everyday contexts, such as when you tell a friend that you’re stuck in traffic and so he or she should expect your arrival in twenty minutes. But expectation is also an important term of art that academics use (sometimes interchangeably with “expectancy”), having been popularized by Dr. Irving Kirsch, who now serves as Associate Director of the Program in Placebo Studies at Harvard Medical School.

Kirsch’s work built on that of Dr. Henry Beecher, who served with the American military during World War II. While deployed in North Africa and Italy, he gave saltwater to wounded soldiers, but told them they were receiving a powerful painkiller. Beecher did not engage in this deception by choice, but by necessity: As an anesthesiologist treating a flood of battlefield injuries, he faced the difficult task of rationing his supply of morphine.

The roots of our understanding of the nocebo effect are more obscure. But we do find an early precedent involving the work of eighteenth-century German physician Franz Mesmer, best known for his interest in the eponymous proto-hypnotic therapy known as “mesmerism.” In the salons of Paris and Vienna, he promoted the idea that illnesses could be alleviated by using magnets to govern the flow of fluid in patients’ bodies. (If this sounds like obvious quackery, which it is, bear in mind that Mesmer lacked any of our modern-day tools of science. He lived in an era when bloodletting with leeches was still seen as state-of-the-art medical treatment.)

Louis XVI (yes, the French king of guillotine fame) learned of Mesmer’s claims, and (properly) regarded them with skepticism. He established a commission to investigate, led by none other than Benjamin Franklin, who was then serving as the United States Minister to France. The American polymath and Francophile performed what we would now refer to as placebo-controlled studies so as to (as the commission put it) “separate the effects of the imagination from those attributed to magnetism.”…

… The mind’s unfortunate ability to create suffering ex nihilo can sometimes affect large groups of people though a process of social contagion (or, in the more indelicate language of the past, hysterical contagion). One such example, known as “The June Bug,” occurred in a U.S. textile mill in 1962. Many employees began to feel dizzy and nauseous. Some vomited. Rumors of a mysterious bug that was biting employees began to circulate, and eventually 62 workers became ill. Yet a subsequent Centers for Disease Control and Prevention investigation determined that no bugs could be identified. Nor could investigators find any other physical cause of the illnesses. This type of phenomenon is now referred to as psychogenic illness—sickness caused by belief.

Over the course of history, there have been countless other examples of psychogenic illness, with symptoms ranging from hysterical laughter to seizures. Aldous Huxley, the famed author of Brave New World, described one such seventeenth-century example in his lesser-known historically-based novel, The Devils of Loudun. In the 1630s, as Huxley documents, an entire convent of Ursuline nuns in the western French community of Loudun became convinced that they’d been demonically possessed (complete with convulsions, and other symptoms recognizable to any connoisseur of the modern exorcism-themed horror-movie genre) due to the unholy machinations of a (genuinely licentious) local priest named Urbain Grandier.

Could such a mass outbreak occur today, in an era when few believe in demonic spirits? Consider that during 2016 and 2017, no fewer than 21 American diplomats serving in Cuba reported a range of bizarre neurological symptoms that later came to be collectively described as “Havana Syndrome.” News of the outbreak spread globally through American diplomatic networks, and eventually more than 200 U.S. diplomats became ill. One leading theory was that the Russian government was attacking American embassies and consulates with microwaves.

To be clear: We do not yet know for certain the cause of these ailments. And it is conceivable that speculation concerning Russian involvement may prove correct (even if the microwave theory is far-fetched). That said, the possibility of psychogenic effects is obvious, and I regard it as concerning that this theory seems to have been rejected out of hand by American officials.

In 2021, in fact, a senior State Department official who’d been mandated to oversee the task force investigating Havana Syndrome was pushed out of her role when she refused to take psychogenic illness off the menu of potential causes. A former C.I.A. officer who claimed he’d been affected by Havana Syndrome while serving in Moscow declared that failing to rule out “mass hysteria” as a cause was “grotesquely insulting to victims and automatically disqualifying to lead the task force.”

I suspect that if Ben Franklin were alive today, he might take a different view…

As we adjust our attitude, we might recall that it was on this date in 1867 that Joseph Lister published the first of his series of articles in The Lancet on “The Antiseptic Principle of the Practice of Surgery.”  Lister, having noticed that carbolic acid (phenol) was used to deodorize sewage, had experimented with using it to spray surgical instruments, surgical incisions, and dressings.  The result, he reported, was a substantially reduced incidence of gangrene.

The biggest component in any human, filling 61 percent of available space, is oxygen. It may seem a touch counterintuitive that we are almost two-thirds composed of an odorless gas. The reason we are not light and bouncy like a balloon is that the oxygen is mostly bound up with hydrogen (which accounts for another 10 percent of you) to make water — and water, as you will know if you have ever tried to move a wading pool or just walked around in really wet clothes, is surprisingly heavy. It is a little ironic that two of the lightest things in nature, oxygen and hydrogen, when combined form one of the heaviest, but that’s nature for you. Oxygen and hydrogen are also two of the cheaper elements within you. All of your oxygen will set you back just $14 and your hydrogen a little over $26 (assuming you are about the size of Benedict Cumberbatch). Your nitrogen (2.6 percent of you) is a better value still at just forty cents for a body’s worth. But after that it gets pretty expensive.

You need about thirty pounds of carbon, and that will cost you $69,550, according to the Royal Society of Chemistry. (They were using only the most purified forms of everything. The RSC would not make a human with cheap stuff.) Calcium, phosphorus, and potassium, though needed in much smaller amounts, would between them set you back a further $73,800. Most of the rest is even more expensive per unit of volume, but fortunately only needed in microscopic amounts.

Thorium costs over $3,000 per gram but constitutes just 0.0000001 percent of you, so you can buy a body’s worth for thirty-three cents. All the tin you require can be yours for six cents, while zirconium and niobium will cost you just three cents apiece. The 0.000000007 percent of you that is samarium isn’t apparently worth charging for at all. It’s logged in the RSC accounts as costing $0.00.

Of the fifty-nine elements found within us, twenty-four are traditionally known as essential elements, because we really cannot do without them. The rest are something of a mixed bag. Some are clearly beneficial, some may be beneficial but we are not sure in what ways yet, others are neither harmful nor beneficial but are just along for the ride as it were, and a few are just bad news altogether. Cadmium, for instance, is the twenty-third most common element in the body, constituting 0.1 percent of your bulk, but it is seriously toxic. We have it in us not because our body craves it but because it gets into plants from the soil and then into us when we eat the plants. If you are from North America, you probably ingest about eighty micrograms of cadmium a day, and no part of it does you any good at all.

A surprising amount of what goes on at this elemental level is still being worked out. Pluck almost any cell from your body, and it will have a million or more selenium atoms in it, yet until recently nobody had any idea what they were there for. We now know that selenium makes two vital enzymes, deficiency in which has been linked to hypertension, arthritis, anemia, some cancers, and even, possibly, reduced sperm counts. So, clearly it is a good idea to get some selenium inside you (it is found particularly in nuts, whole wheat bread, and fish), but at the same time if you take in too much you can irremediably poison your liver. As with so much in life, getting the balances right is a delicate business.

Altogether, according to the RSC, the full cost of building a new human being, using the obliging Benedict Cumberbatch as a template, would be a very precise $151,578.46. … That said, in 2012 Nova, the long-running science program on PBS, did an exactly equivalent analysis for an episode called ‘Hunting the Elements’ and came up with a figure of $168 for the value of the fundamental components within the human body…

As we take our vitamins, we might we might send dynamically-evolved birthday greetings to Stephen Jay Gould; he was born on this date in 1941.  One of the most influential and widely read writers of popular science in his generation (e.g., Ever Since Darwin, The Panda’s Thumb), Gould was a highly-respected academic paleontologist, evolutionary biologist, and historian of science.  With Niles Eldridge, he developed the theory of “punctuated equilibrium,” an explanation of evolution that suggests (in contrast with the gradualism that was prevalent until then) that most evolution is marked by long periods of evolutionary stability, which are interrupted– “punctuated”– by rare instances of branching evolution (c.f., the Burgess Shale).

Scientists have power by virtue of the respect commanded by the discipline… We live with poets and politicians, preachers and philosophers. All have their ways of knowing, and all are valid in their proper domain. The world is too complex and interesting for one way to hold all the answers.

“I’m not afraid of death; I just don’t want to be there when it happens”*…

 

From Sunbelt seekers and snowbird retirees to economic immigrants and political refugees, folks flock to southern Florida.  And many of them chose to return home– if not during their lives, then afterwards…  So it’s no surprise that the Miami area is the U.S. capital of corpse repatriation:

The transnational city is a place to die for. Ironically, however, once established in the transnational city, few envision staying there until their last breath. For many, it is a temporary venue, whether as a place of exile, a springboard for upward mobility, or a playground until new opportunities beckon. Few imagine dying there and, as the moment draws near, many make plans to go home.

In the transnational city, which is home to a disproportionate number of the foreign-born and expatriates, death and repatriation are a steady business. The bodies of an estimated 20 percent of South Florida’s deceased are shipped out, more than from any other region in the USA. Most of the HRs (industry shorthand for human remains) going abroad depart from Miami International Airport. According to the CEO of Pierson, a leader in this business since 1964, around 80 percent of business is international, with the company shipping to a range of foreign destinations across Central and South America and a number of European countries as well…

As we wonder if there’s a discount fare, we might recall that it was on this date in 1914 that “Little Willie,” the first prototype of the British Mark I tank– thus, the first completed tank prototype in the world– rolled out of the shop.  It weighed 14 tons, required rear steering wheels (so got stuck in trenches), and managed only two miles per hour; still, it was the first step toward a technology that revolutionized battlefields.
